* If you wish to discuss your financial commitment with the Dues Committee, please contact Wendy Rudder at 255-0436.  Forms will not be accepted with less than the minimum membership fee without prior arrangements with the Dues Committee.

 

Initial payment is due with this form by August 27, 2007.  To make quarterly payments, divide (E) by 4.  Remaining balance will be billed each quarter.  Make checks payable to “The Jewish Congregation of New Paltz”.
